Multiple K-Wires Fixation of the Intercondylar Fracture of Humerus in Adults

Abstract
As the fractures ofintercondyle of humerus in adults are usually comminuted and involves articular cartilage, the accurate anatomical reduction and rigid internal fixationare requered. Internal fixation, consisting usually of an array of screws.
Kirschner wires, or plate with screws, has been attempted by numerous surgeons.
The fractures of intercondyle of the humerus in our experience were usually so osteoporotix and severely comminuted, that rigid fixation could not be adequately achieved. We used the multiple pinning method with minimal surgical exposure and
minimal
soft tissue dissection.
The authors analysed the twenty five cases of the fracture of intercondyle of the humerus, which were treated at the Ewha Womans University Hospital from Jan. 1987 to Nov. 1991, adn the following results were obtained. Among the twenty five
cases,
the
ratio of female to male was 17:8, and fifteen cases older than 6th decades and the most common cause of injury was falling down(18 cases). The type IV(18 cases) was most common by Riseborough & Radin classification followed by the type III(7
cases). All
were using transolecranon approach and average period of immobilizaton was 5 weeks. The average bony union time wasx 10 weeks and the range ofmotion was average 20 degrees of extentsion lag to average 1120 degrees of further flexion. The results
were
evaluated by the criteria of Riseborough & Radin. At a mean follow-up of 2.8 years, sixteen results were rated as good; seven, as fair; two, as poor. Complications included severely limite motion in two patients and one myositis ossificans. From
this
results, treatment using multiple K-wire fixation is simple, and achieving relative rigid fixation and achieving good range of motion for daily living life.
